Terms of Service Waiver of Liability and Assumption of Risk and Release

By using any service provided by Dog Life Pro LLC,  you are agreeing to the following:

You acknowledge that you understand that attendance by you and/or your dog at a private training session, dog boarding, dog walking, dog daycare or use of the K9 Playtime database is not without risk to yourself, to anyone who may attend with you, your dog(s), and your property, since there may be situations and dog(s) to which you or your dog will be exposed that may be difficult to control and may be the cause of injury to persons, dogs, or property even when handled with the greatest amount of care and precaution.

You acknowledge that you assume all risks and full responsibility for yourself, your dog(s), property and for anyone attending any service offered by Dog Life Pro LLC, with you.  You further agree to release and hold harmless Dog Life Pro LLC, their agents, volunteers, contractors, the training facility and its property owners and management for any illness, injury, or loss that you incur to yourself, to your dog(s), property and to anyone attending with you.

You, as the owner of the dog(s), hereby agree to secure or provide a proper harness and leash as directed by Dog Life Pro LLC,  and to wear proper clothing and shoes to permit effective training of your dog.  You agree to provide proof of vaccinations if requested and understand your dog may not be admitted to daycare or boarding without such proof.   You agree and pledge to abide by the statements in this waiver and will ensure the same of your companions.  You further agree and pledge to abide by all of the terms and conditions set forth in this agreement and terms of service.

You understand that the degree to which a dog is successfully trained is a function of the interest, commitment, and cooperation of  you, the owner/handler.   You acknowledge and agree that there is no guarantee of what your dog will achieve in training based on the instruction given, but that you as the owner/handler of your dog(s) are responsible for teaching and training your dog(s) in accordance with the instruction provided.

By submitting your request for services, you acknowledge that you have read this “Terms of Service, Waiver of Liability and Assumption of Risk and Release”  and agree to its terms below. You also acknowledge that all information is complete and accurate to the best of  your knowledge.

Special Conditions Applying To All In-Home Training

If you feel that your dog is showing signs of aggression, it has bitten another person or animal then please relay this information to us over the phone when we discuss your case. We’ll make a determination on how it is best to proceed. We may ask you to meet us outside with or possibly without your dog. While we do not provide training for aggression we can help you with a very important aspect of handling aggression called management. Management is critical to keeping you and your family safe, and getting your dog on the path to behavior modification. In fact, behavior modification will continually regress without management because aggressive behavior is self-reinforcing.

All in-home training sessions are scheduled for one hour, but may run over at the rate specified in the Services and Rates page on a per minute basis. In addition to the cost of services, travel charges may apply for some locations, but we will make you aware of any travel charges when we set up the appointment on the phone.

Training is the process of teaching the dog’s owner the techniques needed to accomplish the desired goals. Success is dependent upon the owner regularly working with his or her dog and carefully adhering to the instructions and techniques which are demonstrated by the trainer.  

Cancellations

Canceled walks, daycare and training with less than 24 hrs notice will be billed at the full rate. There is no charge for cancellations with more than 24 hrs notice for walks, daycare and training. For boarding cancellations I require one week’s notice. Less than one week’s notice will result in a fee 50% of the value of the entire stay. Less than 24 hrs notice is billed at 100%. 

Your Information

a. Fulfill your order accurately and quickly.
b. Pre-fill information fields for you to save you time and hassle.
c. Identify ways we can improve our site to enhance your visit.
d. Ensure we give proper credit to those who referred you to our site.

What information does DogLifePro.com gather?

During your visit to our website, you may have the option of providing the following information that is stored in our systems:

Your user identification and password that provide access to special areas of our website (if such an ID or password has been issued to you).

Certain information you enter so you only have to enter them once upon future visits to our website, but never credit card information, unless you request that we do.

The contact information you provide including phone, fax, pager, email, addresses and the like.

If you elect to participate in our e-mail programs, you may opt out at any time.

Data you enter into an application that’s necessary to process the form.

Responses to blogs or ePolls are collected in aggregate, but your personal choices are not stored. For example, we will note that 80 percent of our visitors made a particular selection; however, our systems will not note which visitors comprise this group.

Information DogLifePro.com does not gather?

If you are a customer, we do not store your credit card or other payment information on our systems unless you specifically ask us to do so.   Choices you make while viewing our presentation lead you to hallways of interest within the presentation, but your choices are not stored on our systems.  Any additional information you do not provide.

How does DogLifePro.com remember information?

DogLifePro.com uses a technology called “cookies” to remember you and the information you are saving, such as your shipping address. When you visit DogLifePro.com, we send you back a cookie, a small text file that includes a temporary inquiry number, your IP address, plus any items you’ve chosen. Your web browser has a special folder on your computer where it keeps cookies. Cookies cannot harm your computer in any way.  Only the website that places a cookie on your computer can read it; no other website may access the cookie DogLifePro.com places on your computer.

As with all web usage, it’s important for you to not share your personal code or password with anyone else (if either has been issued to you).  With that information, someone could logon as you.   Although DogLifePro.com never displays your credit card information on our website, someone logging on as you could access less confidential data, such as your name or shipping address.

What general information does DogLifePro.com track?

Our site tracking program collects aggregate information about the number of visitors, types of browsers used, Internet Service Providers used, visit times and length, pages visited, referring pages and exit pages.  This information helps us enhance our site so that our visitors have a better experience.

Can I change or remove my information from DogLifePro.com's System?

All visitors have the opportunity to change personal information such as shipping addresses online. To cancel your membership or status email us with those instructions.
